Heart disease is the largest driver of medical costs in the U.S. and remains one of the hardest areas to improve. The Healthcare Transformation Task Force and Chamber, a cardiology value-based care enablement company, co-developed a case study showcasing how Chamber’s model supports cardiologists to prevent hospitalizations, improve outcomes, and reduce total cost of care. This publication coincides with American Heart Month, recognizing the importance of advancing high-quality, prevention-focused cardiovascular care.

Chamber primarily contracts with Medicare Advantage plans, and offers flexible financial arrangements – including full risk, cardiac sub-capitation, and pay-for-performance – to support cardiologists in driving meaningful cost and quality outcomes. Chamber’s model demonstrates:

  • Fewer ER use and inpatient admissions
  • Lower cost of care, including a $2,800 reduction in ER admission costs per patient

These findings show how investing in infrastructure, aligning incentives, and empowering physicians can make value-based cardiology care work at scale.
